چکیده
ACUTE LUNG INJURY IS A MAJOR cause of acute respiratory failure in patients who are critically ill and is associated with several clinical disorders, including sepsis, pneumonia, and aspiration. Although lifesaving, traditional ventilation strategies with higher tidal volumes and airway pressures can exacerbate lung inflammation and injury. Acute lung injury produces parenchymal lung damage that is heterogeneous and may place the patient at risk for ventilatorassociated lung injury. When patients are supine, the reduced volume of the nondependent-aerated lung is at risk for alveolar overdistention, and the cyclical ventilation of the dependent lung at low volumes can cause recruitmentderecruitment with subsequent mechanical strain. CARING FOR THE CRITICALLY ILL PATIENT Low-Dose Inhaled Nitric Oxide in Patients With Acute Lung Injury A Randomized Controlled Trial
INHALED NITRIC OXIDE HAS BEEN shown to be a selective pulmonary vasodilator with minimal systemic effects. Nitric oxide has been shown to improve outcome, as measured by the need for extracorporeal membrane oxygenation, in persistent pulmonary hypertension of the newborn. Inhaled nitric oxide has also been shown to improve gas exchange both in animal models of acute respiratory distress syndrom...
متن کاملCARING FOR THE CRITICALLY ILL PATIENT Ventilation Strategy Using Low Tidal Volumes, Recruitment Maneuvers, and High Positive End-Expiratory Pressure for Acute Lung Injury and Acute Respiratory Distress Syndrome A Randomized Controlled Trial
ACUTE LUNG INJURY AND ACUTE respiratory distress syndrome (ARDS, the most severe form of acute lung injury), are potentially devastating complications of critical illness. Arising in response to direct lung injury (eg, pneumonia) or intense systemic inflammation (eg, sepsis), the pathogenesis involves pulmonary edema, diffuse cellular destruction, alveolar collapse, and disordered repair. Morta...
متن کاملPharmacokinetic Behavior of Theophylline following PEEP in Critically Ill Patients with Acute Lung Injury
The effect of Positive End Expiratory Pressure (PEEP) on the hepatic elimination of low to moderate extraction ratio drugs has not been clearly defined. We prospectively investigated the effect of PEEP on the clearance of theophylline in 30 (20 males and 10 females) intubated critically ill adult patients with acute lung injury/acute respiratory distress syndrome (ALI/ARDS). The Mean (±SD) age ...

متن کاملCARING FOR THE CRITICALLY ILL PATIENT Effect of Exogenous Surfactant (Calfactant) in Pediatric Acute Lung Injury A Randomized Controlled Trial
ACUTE RESPIRATORY DISTRESS syndrome (ARDS) was originally termed the adult respiratory distress syndrome because it resembled the clinical picture of infant respiratory distress syndrome (IRDS), and both exhibited hyaline membranes at autopsy. Avery and Mead first reported that lung surfactant quantity and activity were abnormal in infants with IRDS and surfactant replacement has subsequently b...
